Mid-Valley Clinic has recently incorporated a collaborative care model of services within our Family Practice, expanding current mental and behavioral health care within the clinic. The inclusion of this evidence-based model of care for the whole person supports the Mission, Vision, and Values of Mid-Valley Hospital & Clinic.

This Collaborative Care Model is an innovative approach that seeks to provide exceptional, client-centered, solution-focused treatment for individuals within our Family Practice setting. This approach has been recommended as a best practice by the Surgeon General’s Report on Mental Health and the President’s New Freedom Commission.

The treatment team includes the Primary Care Provider, Psychiatric Consultant, Behavioral Health Care Manager, and the client as the expert on their own experience. Routine screening helps identify those who may benefit from mental and behavioral health services, including counseling and medications. In addition, collaborative care allows for referral services to partnering agencies and external resources.

Mid-Valley believes that this whole-person model of care will redefine how services are provided in our community and lends itself to convenient and compassionate care standards in rural settings. All services are provided within the Clinic and can include telephonic and computer-based interactions.
