Insurance

Mid- Valley offers medical, dental and vision to full time and eligible part time employees.

Medical Plan

Medical insurance is provided for employees the first of the month following 90 days of employment. Dependent coverage is available with an out of pocket cost to the employee. The waiting period for dependent coverage varies from the first of the month following 90 days to 10 months after date of hire depending on the plan offered.

Dental Plan

Employees have two options for coverage through Washington Dental Insurance or Delta Dental. The eligibility waiting period varies from the first of the month following 90 days to 10 months after date of hire depending on the plan offered.

Vision Plan

Eligibility for vision ranges from the first of the month following 90 days to 13 months after date of hire depending on the plan offered.

Paid Time Off

Full-time and eligible part-time employees receive:

Annual Leave

Full time employees will accrue between 160 hours and 240 hours based on years of service. Eligible part time employees will accrue hours on a pro-rated basis.

Sick Leave

Full time and part time employees accumulate sick leave benefits at a rate of .04615 hours per each hour worked up to a maximum of 96 hours per year. Employees may elect to convert sick leave hours to vacation hours if they have accumulated at least 720 hours of sick time in their available bank.

Holiday Pay

Employees may elect to draw from their annual leave bank paid time off for 8 national holidays throughout the year.

Retirement

Mid-Valley offers employees the opportunity to participate in the following retirement plans:

403(b)

All employees may contribute a cap less percentage of pay to a 403(b) plan through payroll deductions on a pre-tax basis, up to the maximum amount allowed by the IRS.

Retirement Plan

After one year of employment and you have worked at least 1000 hours, Mid-Valley will contribute an amount equal to 5.0 percent of each participant’s compensation.

Other Benefits

Full-time and eligible part-time employees may receive (based on contract language in effect) the following additional benefits:

Bereavement/Critical Illness Leave

Emergency leave with pay (prorated for part-time employees) up to a total of 40 hours per calendar year may be allowed for critical illness or death in the immediate family.

Jury Duty Leave

Regular full-time and part-time employees who are called to serve on jury duty shall be compensated by Mid-Valley for the difference between their jury duty pay and their normal straight time pay.
